To register for the Tapemark Charity Women’s Golf Event, please complete the application below and click submit. If your application is accepted, you will receive an email invoice that can be paid online or printed and mailed in.
DISCLAIMER: By clicking submit, having elected to participate in the 2008 Tapemark Charity Women’s Golf Event, I hereby irrevocably and with full knowledge of all consequences, waive any and all claims, causes of action and any and all rights or remedies which I may have or which my heirs, executors and administrators hereafter can, shall, or may have against Tapemark Company, Tapemark Charity Pro-Am Golf Tournament, Southview Country Club, the Minnesota PGA or any agent instrumentally affiliated with the Tapemark Charity Pro-Am. Further, I hereby grant full permission to any and all of the foregoing to use for any lawful purpose any photographs, videotapes, motion pictures, recordings, biographical information or any other record of this event.
There is a $50 non-refundable registration fee included in the entry fee.
